Unit costs above represent the average allowed charge for each service. The allowed charge is the amount of submitted charges eligible for payment for all claims according to health plan documents/contracts. Specifically, it is the amount eligible after applying pricing guidelines, but before deducting third party, copayment, coinsurance, or deductible amounts. The allowed charge represents the amount a plan will consider for adjudication for a covered service. The unit costs above include network and non-network services.
In the event of any discrepancy between the information contained in this tool and official plan documents, the latter shall govern in all cases.

HSA's allow you to set aside tax-free dollars that you use to pay for qualified health care expenses. When you enroll in the Consumer Driven Health Plan, you are eligible to enroll in the HSA. You must enroll in the HSA in order to receive Company contributions, even if you are not making contributions. You must also enroll each plan year to continue to receive the HSA benefit.
The 2024 HSA contribution maximums for employer and employee contributions combined are $4,150 for employee only coverage and $8,300 for family coverage.

The FSA allows you to set aside tax-free dollars that you can use to pay for qualified health care expenses. When you enroll in the PPO or PPO Plus plans, you are eligible to enroll in the Health Care FSA. To participate in the Health Care FSA, you must enroll each plan year.
The 2024 Health Care FSA maximum contribution is $3,200.
IMPORTANT NOTE: Both the HSA and FSA plans require you to enroll each plan year. If you are currently enrolled and take no action during the open enrollment period, contributions will end starting July 1.
